Personal Care Attendant Services: Ready Care provides home based personal care attendant services for elders and individuals with disabilities who require assistance to remain in their home. Ready Care provides both agency-based and consumer directed personal care attendant services. Whether you want to mange your own care by scheduling, hiring, and supervising your care providers or you want assistance from the Ready Care office staff, your personal care needs can be met with qualified personnel who believe that you are in control of your care.

An assessment of your care needs will be completed by trained assessors who will determine the level of care you need in order to remain living where you choose. Once the assessment is completed, Ready Care staff will work with you in developing a schedule that meets your service needs. Ready Care will then assist you in selecting a trained staff who will provide you with care as outlined on your service plan.

Respite Services: Respite services are provided to give family members or other primary care providers relief from their care responsibilities of elders and individuals with disabilities. Respite services are provided by trained care providers who can provide companionship, engage in recreational activities or simply provide support so primary care providers can participate in other activities.

Respite services are a Medicaid Waiver service which can be written into an annual Medicaid Waiver plan of care. Respite services can be provided on an hourly or daily basis. Daily respite services must be provided either in your home or in a licensed facility.

Chore Services: Ready Care provides housecleaning services for individuals who require this assistance to remain in the environment of their choice. Ready Care provides these services in individual homes as well as through contracts with the Alaska Housing Finance Corporation.

Chore services are a highly valued service for many consumers and their families. Consumers who live in a clean environment stay healthier longer. Chore workers can clean, shop, run other errands and ensure that the consumers home is kept free of potential safety hazards. Chore services are a Medicaid Waiver service which can be written into an annual Medicaid Waiver plan of care.

Care Coordination:
Care coordination provides comprehensive case management services for individuals receiving waiver services through Medicaid. Ready Care provides care coordination services for all four waiver groups: MRDD, CCMC, OA and APD.

At Ready Care, we believe that care coordination is a cornerstone to the success of a plan of care. Our care coordinators are seasoned workers who possess life experience which they can apply to their care coordination activities. Ready Care care coordinators will meet with you and your planning team to develop a plan of care that consists of regular Medicaid services, waiver Medicaid services and all other supports you have available to you. Your care coordinator will assist you in developing a daily or weekly schedule that outlines your support needs. The care coordinator will then assist you in contacting local providers to broker services. Your care coordinator will then meet with you one time per month to ensure that your plan of care is being implemented to your satisfaction and that your service needs are being met.

Residential Habilitation: Ready Care provides an array of supported living services inclusive of community living skills assessments, family habilitation services, supported living, and in-home support. Ready Care staff provide directed skill building activities to ensure consumers are gaining community living skills or maintaining acquired skills. The emphasis of residential habilitation services is to ensure that consumers are living the life they choose in their home community.

Supported Employment: Ready Care provides long term job coaching assistance for individuals who require support to maintain their employment. Job Coaches conduct task analytical skill training to ensure consumers are performing their jobs to the satisfaction of their employers. Consumers who receive supported employment services from Ready Care are employed in area businesses and earn at or above the minimum wage level.

Supported Employment services are a Medicaid Waiver service which can be written into an annual Medicaid Waiver plan of care. Once your supported employment services have been approved on your plan of care, Ready Care staff will work with you and your employer in developing a schedule that meets your service needs while also meeting the needs of your employer.

Day Habilitation: Day Habilitation services are provided as a means for individuals with developmental disabilities to explore their communities through the use of leisure and recreational activities. Day Habilitation specialists are matched with consumers who share similar interests. Day Habilitation staff have been known to attend local arts events, join in seasonal recreational activities, attend community schools classes and exploring their respective communities with their consumers. Day Habilitation services are a Medicaid Waiver service which can be written into an annual Medicaid Waiver plan of care.

Intensive Active Treatment: Intensive Active Treatment provides for nursing oversight and training for children and adults with complex medical conditions. Ready Care nursing staff is dedicated to the provision of coordinated community based nursing services. Our nursing staff provides training for direct line staff, answers questions for consumers and families, acts as advocates with the medical community and ensures that prescriptive services are carried out with efficiency and care. The Ready Care nursing staff will complete an annual long term care assessment which describes your nursing needs in detail. Nursing staff work closely with other medical providers such as physician’s, occupational therapists, physical therapists and speech pathologists. Nursing staff will assess the quality of care on an ongoing basis and then complete a formal quarterly review process which is shared with all support team members. Ready Care nursing staff are available to answer questions, provide guidance for service providers and to provide hands on training when necessary. These highly skilled staff receive continuing education from the State of Alaska, Department of Health and Social Services to ensure their skills are of the highest degree possible.

Direct Referrals: Ready Care receives direct referrals from State and other entities who are interested in assistance with obtaining and/or maintaining employment. Direct referrals are contracted on an individual client basis with an accompanying authorization, which dictates hours of service and type of service. Typical services are designed to assist clients with securing employment.

Referral Source: Ready Care recognizes that it is but one service option available in our community. As such, Ready Care acts as a referral source for individuals receiving services. Ready Care will maintain information on a variety of community options which can potentially meet client needs.
